The discs are five bolt for single and twin wheels

5 hole discs fit normally? Do I have to remove the hub and bearing to replace it?

Yes all models have 5 bolt hole discs, just the diameter is different and yes you have to remove the bearing/hub, so a good time to get new bearings whilst you're at it.
