I bet it needs injectors.

Drain some fuel out the bottom of the fuel filter and see if it a nice clear blue colour or if its got any grey swarf in it, if it has that's a sign the pumps shot.
Actually with the original problem its up to you whether you drain a bit of fuel don't want you going back to square one.

If the pump is shot your looking at about 600 sovs for injectors, 3-400 sovs for the pump plus fuel system flush and labour. N

You could chance it (if you can find a mk6 tdci in a scrap yard) but they might be just as bad. N

Yes you will need to code them to your van, you can do that with the f-super. N

You can do a leak off test its simple and cheap to do all you need is,

4 x bottles (baby feeding bottles are good)
4 x pipes (if you have the R clip type fuel return pipes got to the junk yard and liberate the pipes and R clips from a mondeo tdci)

Put 1 pipe into each bottle unclip the fuel return pipes from your injectors and clip the ones going into the bottles on, seal the vans leak back pipes with duct tape or something to stop air getting in.

Run the van for 5 mins and see how much fuel is in each bottle, I think it should be 25 - 40 ml any more and the injector is shot, they should all be near enough the same level. N

That's a lot of smoke, did you do the leak back test to see what the injectors health is,
Do all later tdci's have the yellow thing on the fuel line to the right hand side, mime doesn't have that nor that style filter head.
It sounds like mine when running, did your f-super turn up. N

I took it for a little drive around the block the missing clears and so does the smoking once warm, but it keeps cutting out at junctions or slowing down and the squirly orange light comes on just before it cuts out too
