Check under the N/S rear corner, there should be four rubber hoses going up into the van. The two larger ones are for the rear heater, are they getting hot ? If not then you need to get the water flow sorted by flushing and bleeding any air out.

Where is the cold air coming from ? Normally it comes from roof vents if temp control is set to cold and from the side vents if set to hot.
If you set it to hot and the air is still coming from roof vents then the actuator that controls the air flow is spooned - there is usually a clicking noise from the actuator as the plastic gears slip. The actuator lives in the heater/fan box at the rear and ,aparently, it is fairly common for them to fail.
