exhaust smoke coming from under bonnet

Hi all I've got a 2013 fwd transit trend, every now and then when I'm driving I get a diesel smell in the van and when I stop I get exhaust fumes coming out from the engine bay, has anybody got any ideas what it can be, Thanks Kev.
