Forward wheel stud spinning in place - how to replace

Ford Transit 2003 - FWD
2.0 TDCI (125hp)
Left hand drive (I don't know if that changes anything)
5 studs per wheel

Today I wanted to replace the front wheel brake pads and while removing the wheel one of the wheel studs started spinning in place.

1. How do I remove the wheel now? any ideas?
2. How do I remove the spinning wheel stud? Do I need to dissemble the hub / disk? If so, how do I proceed?
3. Which wheel stud size do I have to buy? The wheels are the original ones, unmodified.
4. Can it happen that the spikes in the hub hole (stopping the wheel stud from rotating) are worn?
5. Can I continue driving the car with the stud as it is? (it is extremely hard to budge(turn) it even with a very long wrench - even harder than to undo the other nuts)

Any other pointers are very welcome!

The only way i can think you can remove is so:

take the other 4 nuts off
and get someone to push on the wheel so that it twists and locks against the hole through the wheel. so jamming the stud

other than that take it to a tyre and exhaust place and get them to remove it...as they are used to doing this ..because they have to remove many locking wheels nuts.

not sure ..if you remove the hub, i think it destroys the wheel bearing, so you then need a new bearing before you do this - others will say if I'm right or not.
new hub may come complete with wheel bearings and studs
