MK6 fwd T300 2.0l Duratorq blowing bluey whitey smoke

My MK6 fwd T300 2.0l Duratorq blowing bluey whitey smoke when its reved up and if you drive it when you put your foot down it dies.
the smoke is unburt fuel
it starts on the button. Egr valve is blanked off.
it allways ran sweet
Could it be injectors ? 250k is to the moon in miles
milage 250k allway serviced.
Owned her for 22 years and its been completely rebuilt ,the body last year
like a new van But I have this problem

What fault codes are you getting..??

Fuel filter blocked
Replaced with new filter
Me Forgot to change it on a regular yearly basis
you can pick up dirt from filling station tanks
