ford transit ,year 2002 ,2.0 tddi Duratorq problem

Hello everyone.
I have a problem with my car.In the following lines i will explain to you all the simtomps.When i am on the road with my van and reduce the speed ,lets say for example from 50 mph to aprox. 15 mph in 1st and 2nd at 1000-1500 rpm's,my engine is galloping and i cannot keep the car between these revs 8 (1000-1500).All this galloping is felt mostly on a country road and very little on a good road.Please help my by giving me some advice on this matter.
