How to read and decipher codes

Hi from Australia,
I’m hoping someone can tell me how to read and decipher fault codes on a 1998 2.5lt diesel Transit, I’ve found OBD port in fuse box but unsure of which connections to jumper.

Unfortunately its not a generic code reader :(

The correct tools are Fords FDS2000 or WDS system (IDS might also work on yours as a late one) or the Lucas Laser 2000. Some users have also reported sucess with Forscan (free software download) and one of their recommended adaptors for phone or laptop
