Starter Motor Sticking/ 2001 2.4

I have a 2001 transit RWD, the flywheel & starter were changed in Feb 07 due to the starter failing, again. Had the solid flywheel fitted this time to do away with the problem of swarf etc. The starter motor on two occasions since has failed to dis-engage after starting, this will of course eventually kill the stater & makes a terrible noise, has anyone else had a similar problem, could it possibly be an electrical/wiring fault on the van.
