I'm sorry about my reply above here; I hadn't actually read the post, only the headline, and saw an opportunity to give some of the limited knowledge I had. Maybe, with some luck, someone visiting the post for that type of scenario could stumble upon it in the future ;)

Anyway, I have a suggestion that I don't see listed here (in case the problem should return).
Do you have a trailer electrical connection on your van?
From my experience (non-transit, but think it would still apply), if the trailer electrical connections rust and short circuit or abridge, if say two random cables/lines end up touching somehow, this can cause all sorts of weird behavior to any lights that are part of what gets directed to a trailer, such as brake lights or even fog lights.
Worth inspecting for anyone ever getting similar symptoms :)
Also, there is earth on both sides left+right for the rear lights, in case you find one and it looks good, then rule out the earth before checking other side ;)

Had this myself recently and a brake switch error code.

All 3 brake lights on, constantly.

In my case the NS rear bulb was dislodged/ loose.

I bent the little metal tab up inside the bulb holder so bulb was tighter put the same bulb back in and all been fine since.

Not sure why it was causing it but it solved it, maybe it was picking up the rear light feed. God knows

I believe that he's talking about the two bulbs in the rear light clusters which illuminate when the brake pedal is depressed, not the low fluid warning light on the dashboard.

Showing my age? that's nothing, after spending today with my kids, their wives, and my grandkids, and not being able to join in the conversation, because I have no idea what or who they are talking about most of the time, and also, some cr@ppy awards program came on the telly, and they were incredulous that I had no idea who any of the "celebs" or the presenters were. I countered that with saying that over the weekend, I recognised Rick Wakeman on a celebrity episode of The Chase, and they all asked, WHO?
