Transmission noise and lurching/judder

2002 Mk6 2.4 RWD LWB
Two things going on that may or may not be related:

1. In first/reverse the transmission seems to be on and off a bit making the van lurch along. Sort of pulls then doesn't, on and off if that makes sense.
Higher revs/riding the clutch smooths it out. AFAIK the clutch was replaced around 30k miles ago.

2. I've noticed a subtle whirring noise when cruising, hard to tell where from. Again it's sort of got a steady on/off rhythm.
It might be there at higher speeds but at a pitch masked by engine/road surface noise, wouldn't like to say.
Still there when coasting with the clutch in so probably not that?

Thoughts are:
    Wheel bearing?
    Diff (hope not!)?
    Half shaft?
    Propshaft bearing???

Anything else to consider? Anything I'm barking up the wrong tree at?
