Need help on Fault code PD269D and P20CB

I have the eml on my transit custom 2013. The codes showing are -

P269D - Exhaust after treatment glow plug control circuit low

P20CB - Exhaust after treatment fuel injector ‘A’ control circuit/open

Has anyone experienced these or has any knowledge or pointers.
