i drive a 135 t350 luton and it doesnt get better than 20 mpg......and thats checked monthly over a 2 year period

manual gives 32 average so your not far out, 25-28 on my jumbo 135, what axel you got

blank EGR, that will give gains straight away, use ford fomula e 5w-30 oil, aswell replacing filters, smash cat out (with no EGR it wont be required as much anyway), use some good fuel BP ultimate or shell vpower, and/or some good fuel treatment.
check injector correction factor codes are programmed in correctly this can make a huge difference.
check for split boost pipes, slight leaks can cause turbo to work a lot harder loose efficiency.
check all tyre pressures, check no brakes are sticking on, specially after a good run.
check diff oil,gearbox oil.
